In the event of a medical emergency, I authorise the leaders of Oak Flats Anglican to contact emergency services and arrange medical attention for my child. From that point onward, emergency services will liaise with me and with church representatives. Relevant medical information may be shared with third parties involved in my child’s care as needed.

I understand that every effort will be made to contact me before any medical procedures are carried out, in order to obtain consent. I acknowledge that Oak Flats Anglican is not authorised to approve any invasive medical procedures. In the case of a minor medical incident that does not require emergency services, standard first aid may be administered. I will be contacted, or one of the alternate contacts listed, to advise of the steps taken.
